Concussions due to a new Indiana State Law that went into effect
July 1st, 2012.
The Law states that Schools must inform and educate high school
coaches, student athletes and parents of student athletes on the
nature and risk of concussion and head injury to student athletes,
including the risks of continuing to play after a concussion or head
Each year, before beginning practice for an interscholastic or
intramural sport, a high school athlete and parent must be given
concussion information, sign an acknowledgment receipt form and
return the form to the coach. The athlete will not be permitted to
participate until the signed form is on file.
An athlete suspected of sustaining a concussion must be removed from
play, even during a practice or game. The student must be evaluated
and receive a written clearance to return to play by a licensed
Health Care Provider trained in the evaluation and management of
concussions and head injuries.
The required forms can be found here and also on the Jay County High
