Medication Quick Facts
- All medications given at school must have a note from the
parent/guardian with date, student’s name, name of the medication, amount
to be given, when it is to be given, reason for medication and length of
time the medication will be given.
- Medications should be given at home whenever possible.
Once a day, twice a day and even three times a day medications can
usually be managed at home.
- Non-prescription medications (those bought over-the-counter) must
be in the original package/container, not a baggie, envelope, etc.
Over-the-counter medications will only be given according to the
directions on the package, unless the school receives a doctor’s order to
do otherwise. The school will need a signed note
from the parent/guardian with date, student’s name, name of medication,
amount to be given, when it is to be given, reason for medication and
length of time the medication will be given.
- Prescription medications must be in original container with
pharmacy label or written instructions from the doctor.
Send to school (in the original container) only what is needed for
the doses to be taken at school. Prescription
medications will only be given according to doctor’s written instructions
or the instructions on the pharmacy label. The
school will need a signed note from the parent/guardian with date,
student’s name, name of medication, amount to be given, when it is to be
given, reason for medication and length of time the medication will be
given. Changes in long-term daily medication must
have a written note from the doctor.
- Parents are responsible for assuring medication arrives safely at
school.
- Medication will not be sent home with a student eighth grade or
younger. Ninth thru twelfth grade students may bring
medications home if the school has received written permission from
a parent/guardian. (2001 Indiana State Law)
- After contacting parent/guardian regarding pick-up, or the conclusion
of the school year, all medications not picked up within a timely manner
will be destroyed.
